Every treatment available today, from antidepressants to therapies used around the world, exists because people participated in clinical research. Clinical trials help researchers find out whether new treatments are effective, safe, and meaningful for people living with conditions like major depressive disorder (MDD) or bipolar disorder. I have filled out the initial screener and qualified. What's next?
If you have already qualified to participate in one of the trials above, then you have been referred to SiteRx for medical record retrieval. You will receive an email and/or text message with a link to a DocuSign medical records release form:
The email will come from ‘Site Rx Records’ via DocuSign.
If you don’t see the message, please check your spam or junk folder. It is critical for participation in the trial to complete this step in a timely fashion.
Please agree to receive DocuSign email and/or text notifications so the trial site can continue to communicate with you.
Upon clicking the link to the form, you will be asked to:
- Review and agree to the SiteRx Privacy Policy
- Confirm or update your Physician’s Contact Information
- Authorize SiteRx to retrieve your medical records on your behalf by reviewing and approving the documents we will send to your physicians to retrieve your records
Screening Visit
During this process, you will also be referred to the clinical research site. Once you have completed the medical records release, your local research site will contact you to schedule your first visit.
During this visit, the site team will:
- Review the informed consent form with you in detail
- Confirm your eligibility and collect standard medical assessments
- Explain how treatment is administered under the study protocol
Be sure to bring your photo ID, medication list, any recent lab results and any questions you’d like to discuss with the site team.
