How do I use the Doctor Visit Guide on PatientsLikeMe.com?

The Doctor Visit Guide is a resource that will help you organize your health information for upcoming appointments. In addition, it will help you identify any questions you may want to ask your doctor so that you can get the care that works for you.

How do I access the Doctor Visit Guide?

• Click the MY HEALTH tab
• Click Doctor Visit Guide
• Click Create a new report

Visit Details

Start by completing the required fields, such as the name of the physician, date, and time. Once the required fields have been updated, the Save button will become enabled. 

Purpose of Visit

Select the topics you're interested in discussing during the visit such as conditions, symptoms, and treatments. Include a brief description for each topic, with the ability to insert more in depth questions and concerns throughout the Doctor Visit Guide.

Conditions

Here, you will find the conditions you are tracking using PatientsLikeMe. Select the conditions you'd like to review with your physician or specialist.

Symptoms

The next step allows you to go through the symptoms in your report and determine how far back you'd like the report to show. You can uncheck any symptoms you do not wish to address.

Treatments

You can pick which treatments you'd like to mention here. The listed treatments are linked to your treatment check-ins on the app or the full site, not from the Purpose of Visit section filled out previously. Either way, both entries will be on your final doctor's report.

Questions and Topics

The final step before seeing your report allows you to expand on specific topics, questions, and concerns. If you want, you can leave the text box blank and check off our pre-made questions:

Once you click save you can view and print the full report. You can also delete or edit a report for an upcoming appointment here as well. 

*Note if you make edits, make sure to click Save, then click View Report.

How do I access existing reports?

• Click the MY HEALTH tab
• Click Doctor Visit Guide
• Locate the doctor report
• Click View Report
