What is Level2?
Level2 is personalized and data-driven care for individuals with a type 2 diabetes diagnosis in the last 24 months or are prescribed certain glucose-lowering medications. Through an individualized approach, guided by wearable technologies and personal coaching, Level2 focuses on the reduction of A1C, increasing time in range of blood glucose levels, and the possibility of type 2 diabetes remission.
How are PatientsLikeMe and Level2 working together?
PatientsLikeMe is providing Level2 members support on their health journey by providing a safe space and vibrant community where members can find encouragement from other patients, set health goals, share treatments, track symptoms, and learn from each other.
Who can join Level2?
Members must be a current UnitedHealthcare member with a covered health insurance plan and diagnosed with type 2 diabetes within the past 24 months, or are prescribed certain glucose-lowering medications.* If benefits change, eligibility to participate may also change. Members may choose to end their participation at any time.

*Eligible glucose-lowering medications include sulfonylureas, meglitinides, thiazolidinediones, alpha-glucosidase inhibitors, dopamine-2 agonists, SGLT-2 inhibitors, GLP-1 receptor agonists, DPP-4 inhibitors, and pramlintide. Use of these medications for polycystic ovarian syndrome is excluded. Other exclusion criteria may apply.
What’s the cost?
Level2 is provided at no additional cost to eligible members as part of their UnitedHealthcare covered health insurance plan.
How long can a person participate in Level2?
For as long as they are a current UnitedHealthcare member with a covered health insurance plan. If benefits change, eligibility to participate may also change. Participation in Level2 is completely voluntary and members may choose to end their participation at any time.
Will participation affect a member’s health care coverage or benefits?
Participation (or declining to participate) has no impact on health insurance coverage or benefits. Premiums, deductibles, and copays are not affected by participation in Level2.

What is a Dexcom continuous glucose monitoring system?
The Dexcom continuous glucose monitoring system (CGM) is a small wearable device that can track a person’s glucose levels 24/7 up to 10 days at a time and displays that information on a smartphone.
How does the CGM check a person’s glucose?
The continuous glucose monitoring (CGM) system has a small sensor (about the size of a thread) that sits under the skin and measures glucose (sugar) levels. It sends glucose data wirelessly to a compatible smart device (see https://www.dexcom.com/faq/what-devices-and-software-are-compatible-dexcom-cgm-apps for a list of compatible devices), so a person can track glucose highs and lows at regular intervals and make informed decisions about diabetes management.
How long does the CGM last?
Each sensor can be worn for up to 10 days before it needs to be replaced. In case the adhesive on the patch surrounding the sensor starts to peel, participants are sent reinforcement materials to supplement the stickiness. When 10 days is up, the sensor can be removed from the abdomen and replaced with a new one. The transmitter (the small gray part which is not disposable) is taken out to be attached to a new sensor. The old sensor can be thrown away.
How does a member get more CGM sensors and transmitters?
Level2 manages orders and refills of CGM sensors and transmitters so members don’t have to worry about it. Members can get in touch with their coach or call the Level2 support line at 1-844-302-2821 from 8 a.m. to 8 p.m. CT Monday through Friday to get more CGM supplies.
Can you join Level2 without a continuous glucose monitoring system?
No, the CGM is an essential part of Level2 and they are not currently enrolling members who cannot wear a CGM or are without compatible smartphones. Level2 is actively working with their device vendor partners to expand the number of compatible smart devices.

What happens if devices provided by Level2 are lost or damaged?
The lost or damaged items will be replaced by Level2. If a device has been lost or damaged, call the Level2 support line at 1-844-302-2821 from 8 a.m. to 8 p.m. CT Monday through Friday.
